You are welcome to use your HSA/FSA to purchase your prescriptions.
If you feel that you would like to submit a claim to your insurance, Apothicare 360 Pharmacy can provide you with a universal claim form to submit for a reimbursement, but please understand the majority of insurance companies DO NOT reimburse for compound medications and they will not reimburse if the pharmacy does not have a contract with them.
The Universal Claim form is provided complimentary to you at your request.
